A potentially serious complication of IVF in which the ovaries overrespond to stimulation medications, becoming swollen and leaking fluid.

Mild OHSS (bloating, mild pain) is common after retrieval. Severe OHSS (rapid weight gain, difficulty breathing, reduced urine output) requires hospitalization. Risk is higher in patients with PCOS or high AFC. Using a GnRH agonist trigger and freeze-all strategy significantly reduces OHSS risk.
